School-based Dental Services
Our portable dental service travels throughout Alleghany, Ashe and Watauga Counties serving basic dental needs in the community. We visit schools and community locations providing preventive and restorative dental services like cleanings, x-rays, exams, fluoride, sealants and fillings. For services beyond these, we are able to refer to our dental office in Ashe County or to another provider.
School-based Dental Care Forms
If you are a parent or guardian of a child who attends local public school in Alleghany, Ashe or Watauga Counties and would like your child to be seen at school through our portable dental program, please complete the enrollment information (linked below) and return it to our office or email it to dental.services@apphealth.com.
